I, the above signed, do hereby voluntarily submit to allow my son/daughter to stay at the Universal kempo-Karate Mountain View Branch to take part in the "VALENTINE'S NIGHT OUT". I do hereby relese and discharge Universal Kempo-Karate Schools Association, it's owners, agents, sponsors, employees and instructors from all claims for injuries that my son/daughter may sustain or incur and from any cause of actions that may arise from their attendance or participation in the aforesaid "VALENTINE'S NIGHT OUT". I understand that medical treatment given my child will be of first aid type only.
